Location: Jobsite    On or about February 5, 2020, employees were exposed to hazards associated with striking an underground natural gas service, such as explosions, burns and an oxygen desiccant atmosphere , during excavation where utilities were unmarked.

Location: Jobsite    1. On or about February 5, 2020, employees were exposed to cave in hazards while working in an excavation with straight cut walls,  at depth of approximately 7 feet without the use of a protective system.    2. On or about February 19, 2020 employees were exposed to cave in hazards while working in the same excavation where a protective system had not been installed after several assurances that it would be abated.
