29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about February 5, 2020, employees were exposed to a struck by hazard while performing framing activities on a new apartment building located at 41 French Mill Run, Cuyahoga Falls, Ohio 44223. The employees were using air powered nail guns and the employer did not ensure that they were wearing protective safety glasses.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about February 5, 2020, employees were exposed to a fall of approximately 20.5 feet while performing framing activities on a new apartment building located at 41 French Mill Run, Cuyahoga Falls, Ohio 44223. The employees were working on all areas of the upper level and the employer did not ensure that they were using fall protection.
