29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6)    a) On or about November 26, 2019, the employer did not ensure that energy control application steps to control the electrical energy source were implemented prior to maintenance and servicing tasks being performed on the 6540 slitting head.  Employees were exposed to crushing hazards between the slitter rollers when wrapping carpet around the slitter head to cover the knives while eneregized.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:    a) On or about November 26, 2019, the employer did not ensure that employees were trained as authorized employees prior to performing set up tasks on the 6540 slitting head.  Employees were exposed to crushing hazards between the slitter rollers when wrapping carpet around the slitter head to cover the knives while eneregized.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) On or about February 5, 2020, the employer did not ensure the slitter 6540 was adequately guarded to prevent employees from struck-by and crushing hazards.  It was found that the interlock on the door at the entry table/slitter area was broken and not connected correctly.
