29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.      a) All Star Bagel New Prospect: The employer did not provide and require employees to use personal protective equipment, such as, but not limited to, a face shield or goggles, when pouring and cleaning with The Home Store Bleach (corrosive),  on or about 2/5/20.    NOTE: THE EMPLOYER IS EQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OR $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:     a) All Star Bagel New Prospect: An eye wash station capable of providing 15 minutes of continuous flushing was not  provided where employees used corrosive chemicals including, but not limited to, The Home Store Bleach, on or about 2/5/20.     NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATMENT CERTIFICATION FOR THIS ITEM FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
