29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected from falling by the use of personal fall arrest systems or guardrail systems meeting the requirements of (g)(4) of this section: a) Complete guardrail system nor personal fall protection systems were used when working on a scaffold. Midrails were not provided on the scaffold platform at the edge of the steep roof employer was repairing; fall hazard/ elevation was 45 ft.
