29 CFR 1910.132(b): Employee-owned equipment. Where employees provide their own protective equipment, the employer shall be responsible to assure its adequacy, including proper maintenance, and sanitation of such equipment. (a) On or about February 6, 2020, the employer did not assure that employee owned noise cancelling Bluetooth headphones met the requirements for hearing protection under the OSHA noise standard, as those employees were exposed to noise levels up to 91.0 decibels or 115.1% of the daily dose in an eight hour shift.

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. (a) On or about February 26, 2020, at the coining machine table, a power strip was attached to the pegboard at the back of the table with zip ties, and had several cords plugged into it, including an extension cord and computer components.

29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. (a) On or about February 6, 2020, the roller machine guards were not secured with a tool to prevent employees from opening them when the machines were in operation.
