29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition:  At a worksite located at 111 W Fountain Rd, Webb City, MO: Employees were exposed to possible severe burns from the potential fire hazards from the accumulation of flammable dust.   a) This was most recently documented on February 6, 2020, when feed dust had accumulated on elevated surfaces in the bagging area inside the enclosed building. Dust had accumulated on the surfaces of walls, wall ledges, and on top of equipment and other fixtures in the work area. The employer failed to ensure that surfaces inside the bagging area were being kept clean of feed dust which could create potential fire hazards.    b) This was most recently documented on February 6, 2020, when feed dust had accumulated on elevated surfaces in the mixing area inside the enclosed building. Dust had accumulated on the surfaces of walls, wall ledges, and on top of equipment and other fixtures in the work area. The employer failed to ensure that surfaces inside the mixing area were being kept clean of feed dust which could create potential fire hazards.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system:    At a worksite located at 111 W Fountain Rd., Webb City, MO: Employees were exposed to fall hazards. The employer has failed to ensure that employee are protected from falling while working on surfaces that were elevated over 4 feet. This was demonstrated on February 6, 2020 when an employee was permitted to stand on top of a ribbon mixer (Mixer #1) to reach the totes they are emptying. The top of the mixer has unprotected sides or edges. The top of the mixer is located approximately 6 feet above the concrete floor and contain no railings or guard rails.

29 CFR 1910.29(b)(1): 29 CFR 1910.29(b)(1): A top rail, or equivalent guardrail system member, 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface was not installed:    At a worksite located at 111 W Fountain Rd., Webb City, MO: Employees were exposed to fall hazards. The employer is failing to ensure that employee are protected from falling while working on surfaces that were elevated over 4 feet. This was demonstrated on February 12, 2020 when an employee was permitted to use the ladder to reach a platform located in the grinding area with an unprotected sides or edges. This platform is located approximately 8 feet above the concrete floor and contains short railings that only come to the employees waist (38 inches). In addition, on the ladder side of the platform there is nothing in place to prevent a worker from falling backwards while they are checking the magnet.

The employer is failing to protect employees by not following the listed or labeled of electrical equipment use to power machinery.  a) This was demonstrated on February 6, 2020 in that a relocatable power tap was connected in series (daisy chained) to a second relocatable power tap to power the open top bagging machine and sewer. The employer is failing to protect employees by allowing relocatable power tap to be both zip tied to the equipment frame and daisy chained into another relocatable power tap.  b) This was demonstrated on February 6, 2020 in that a flexible cord (extension cord) was connected in series (daisy chained) to a second flexible cord that powered a sewer machine near the grinder. The employer is failing to protect employees by allowing multiple extension cords to be plugged in instead in lieu of permeant wiring.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  At a facility located at 111 W Fountain Rd, Webb City, MO: Employees were exposed to electrical shock hazards. At the time of the inspection, each outlet box in completed installations did not have a cover. The following outlet boxes were missing covers:   a) This was demonstrated on February 6, 2020 in that a switch cover had been removed from behind the open top bagging machine and sewer. The employer is failing to protect employees by permitting employees to operate the equipment with exposed wiring.   b) This was demonstrated on February 7, 2020 in that a switch cover was missing in the bathroom. The employer is failing to protect employees by permitting employees to plug in a space heater into the receptacle without an outlet cover.  c) This was demonstrated on February 7, 2020 in that an operator panel for the grinding machine contained wiring that was exposed. The employer is failing to protect employees by permitting employees to operate the equipment in such an exposed fashion.

This was demonstrated on February 6, 2020 in that flexible cords and cables with the outer jacket on the 110-volt power cord that runs the open top bagger and sewer were withdrawn, exposing the inner conductors and terminals to strain. The employer is failing to protect employees by permitting the use of the cords and cables in the relocatable power tap that are damaged.

The employer is failing to protect employees by not warning them of the danger imposed by the equipment space with a sign reading DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign.
