29 CFR 1910.67(c)(2)(v): (v) A personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, shall be worn and attached to the boom or basket when working from an aerial lift.      A) Exterior, in lift:  An employee was working in a Terrex lift and was not tied off.  The employee was exposed to fall hazards of approximately 18 feet, on or about January 29th, 2020.       NOTE: Because abatement of this violation is already documented in the inspection case   file, the employer need not submit certification or documentation of abatement for this   violation as normally required by 29 CFR 1903.19.

29 CFR 1910.133(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.                                                                                          A) Exterior, In the bucket: An employee was exposed to eye/face injuries while using a chain saw and was not protected with any type of eye/face protection, on or about January 29th, 2020.    B) Exterior, On the ground: Employees were exposed to eye/face injuries while feeding branches into a wood chipper and were not protected with any type of eye/face protection, on or about January 29th, 2020.      NOTE: Because abatement of this violation is already documented in the inspection case  file, the employer need not submit certification or documentation of abatement for this  violation as normally required by 29 CFR 1903.19.

29 CFR 1910.135(a)(1): The employer shall ensure that each affected employee wears a protective helmet when working in areas where there is a potential for injury to the head from falling objects.      A) Exterior, in the bucket: An employee was exposed to head injuries while working in a bucket cutting branches. The employee was not protected by a hard hat or protective helmet, on or about January 29th, 2020.      B) Exterior, On the ground: Employees were exposed to head injuries while working on the ground below an employee in a bucket cutting branches. The employees were not protected by hard hats or protective helmets, on or about January 29th, 2020.     NOTE: Because abatement of this violation is already documented in the inspection case   file, the employer need not submit certification or documentation of abatement for this   violation as normally required by 29 CFR 1903.19.
