29 CFR 1926.100(a):       Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    A) Exterior, On the scaffold: An employee was exposed to head injuries while working below another employee above. Both employees were masonry/siding activities on the scaffold. The employee below was not protected by a hard hat or protective helmet, on or about December 5th, 2019.      NOTE: Because abatement of this violation is already documented in the inspection case  file, the employer need not submit certification or documentation of abatement for this  violation as normally required by 29 CFR 1903.19.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.      A) Exterior, On the scaffold: Employees were exposed to fall hazards of approximately 12 and 18 feet while climbing the scaffold components and were not provided with a safe an acceptable means of access, on or about December 5th, 2019. A) Exterior, On the fabricated framed scaffold: Employees were exposed to fall hazards of approximately 12 and 18 feet while working on different levels of a fabricated framed scaffold that were not protected by a complete guardrail system (including end rails) or another means of fall protection in place, on or about December 5th, 2019.      Employees must be protected from falls by the use of a guard rail system (including end rails), a personal fall arrest system or some other means of fall protection.          NOTE: Because abatement of this violation is already documented in the inspection case  file, the employer need not submit certification or documentation of abatement for this  violation as normally required by 29 CFR 1903.19.
