29 CFR 1910.219(b)(1): Flywheels. Flywheels located so that any part is seven (7) feet or less above floor or platform shall be guarded in accordance with the requirements of this subparagraph: a. On and/or before February 6, 2020, on the MD4 line in the press department, the access opening on the flywheel of the Crown multi-die press 4 (model: Crown 64; S/N: 62H-139) was not guarded due to a cover that was open and not secured, thereby exposing employees to a struck-by and/or caught-in hazard.
