29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee   training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) On or about November 7, 2019 - at the above addressed jobsite - Production Area, the employer did not ensure an energy control program consisting of an energy control procedure, employee training and periodic inspections was established for employees performing service and maintenance on machines including, but not limited to, the CC-1.5 Tortilla Chip Fryer,  thereby exposing employees to hazards associated with rotating and moving parts.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a) On or about November 7, 2019 - at the above addressed jobsite, facility wide, the employer did not ensure that employees who operate the Mitsubishi - Service Max S2749 / S/N FGC25K were properly trained, evaluated and certified to perform the tasks in a safe manner, thereby exposing employees to struck-by hazards.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:    a) On or about November 7, 2019 - at the above addressed jobsite - Production Area,  the employer failed to provide effective guarding to protect employees working at or near the Tortilla Chip Fryer, Model No.CC-1.5 , Serial No. 10320 that had exposed moving parts, including, but not limited to chains and sprockets.
