Werner portable extension ladder to gain access to the 5:12 pitch roof to perform roofing activities. The portable ladder used for access to the upper landing surface and the ladder's length, the ladder side rails did not extend at least 3 feet (.9 m) above the landing surfacing being accessed on or about February 10, 2020. ABATEMENT CERTIFICATION REQUIRED
