29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a. At 5316 Golf Links Blvd. in Zephyrhills, FL on or about January 17, 2020:  Employees engaged in the removal and replacement of concrete tiles were exposed to a 27-foot fall hazard, in that, fall protection equipment was not installed or utilized.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a. At 5316 Golf Links Blvd. in Zephyrhills, FL, on or about January 31, 2020:  Employees engaged in the removal and replacement of concrete tiles on a 6 in 12 roof were exposed to a 27-foot fall hazard, in that the employer failed to provide training to each employee exposed.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.    a. On or about January 17, 2020 at 5316 Golf Links Boulevard in Zephyrhills, Florida, an employee suffered a work-related injury and was hospitalized.  The employer was aware of the in-patient hospitalization on January 17, 2019 around 12:00 PM.  The employer notified OSHA on January 23, 2020 at 4:47 PM of the in-patient hospitalization.
