29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:     (a)  GH Stanley Management, dba The Stanley Hotel, 333 E Wonderview Avenue, Estes Park, CO: On and preceding 2/10/2020, the employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  Employees used Auto-Chlor System GOK Grill & Oven Kleen (containing 10-15% potassium hydroxide) to clean grills, fryers, and ranges.  GOK Grill & Oven Kleen was mixed in a bucket with water, applied to surfaces, and then wiped off.  GOK Grill & Oven Kleen was a corrosive liquid.  Eye protection was not worn by employees.  The employer did not ensure that employees used appropriate eye protection.  This condition exposed employees to a chemical hazard.

29 CFR 1910.138(a):  The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes:  (a)  GH Stanley Management, dba The Stanley Hotel, 333 E Wonderview Avenue, Estes Park, CO: On and preceding 2/10/2020, the employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from chemical burns.  Employees used Auto-Chlor System GOK Grill & Oven Kleen (containing 10-15% potassium hydroxide) to clean grills, fryers, and ranges.  GOK Grill & Oven Kleen was mixed in a bucket with water, applied to surfaces by hand, and then wiped off using a rag. GOK Grill & Oven Kleen was a corrosive liquid.  Employees wore disposable nitrile food service gloves.  Disposable food service gloves do not provide adequate protection from exposure to corrosive liquids.  The employer did not ensure that employees used appropriate hand protection.  This condition exposed employees to a chemical hazard.  (b)  GH Stanley Management, dba The Stanley Hotel, 333 E Wonderview Avenue, Estes Park, CO: On and preceding 2/10/2020, the employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from thermal burns.  Employees handled hot cookware on the gas range, grill, and fryer.  Employees used kitchen towels to handle hot cookware.  The employer did not ensure that employees used appropriate hand protection.  This condition exposed employees to a thermal burn hazard.

29 CFR 1910.1200(h)(1): Employer had not provided employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees had not previously been trained about was introduced into their work area:  (a)  GH Stanley Management, dba The Stanley Hotel, 333 E Wonderview Avenue, Estes Park, CO: On and preceding 2/10/2020, the employer had not provided employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees had not previously been trained about was introduced into their work area.  Employees used chemicals for kitchen tasks.  The employer had not provided hazard communication training to employees.  This condition exposed employees to chemical hazards.  Chemicals included, but were not limited, to the following:  1.    Sysco Classic Germicidal Ultra Bleach;  2.    Auto-Chlor System GOK Grill & Oven Cleaner; 3.    Auto Chlor System Bio Floor; 4.    Auto Chlor System bleach; and 5.    Auto Chlor System Solution Q.A. Ultra Sanitizer.

Operations were conducted in the kitchen that exposed employees to hazards, such as but not limited to corrosive liquid hazards when using oven cleaner, thermal hazards when handling hot cookware, thermal hazards when cleaning hot ranges, grills, or fryers, and chemical splash hazards when using kitchen cleaners, such as bleach.  The employer had not certified that a workplace hazard assessment had been performed.  This condition potentially exposed employees to chemical and thermal hazards.

29 CFR 1910.305(a)(2)(xi): Support for cable assemblies and flexible cords and cables were not installed so as not to cause damage:  (a)  GH Stanley Management, dba The Stanley Hotel, 333 E Wonderview Avenue, Estes Park, CO: On 2/10/2020, the employer did not ensure that support for cable assemblies and flexible cords and cables was installed so as not to cause damage.  The flexible power cord for the hot box used to maintain temperature of food, which was located in the hallway outside the kitchen, was plugged into an outlet located approximately 7'2" above floor level.  The insulation around the conductors had separated at the plug.  The employer did not ensure that strain relief was provided for the power cord.  This condition potentially exposed employees to an electrical hazard.
