29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  At the time of the inspection, employees exposed to noise above 85 decibels for an eight hour time weighted average were not covered by a hearing conservation program including annual audiograms.  The following employees were sampled and exposed to the 8 hour time weighted averages;  1. D.C sampled for 420 minutes with the remaining time counted as zero exposure received an 8 hour exposure Dose of 135.6 equivalent to an average of 92.2 decibels.  2. J.O. sampled for 426 minutes with the remaining time counted as zero exposure received a 8 hour exposure Dose of 107.4 equivalent to an average of 90.5 decibels.  3. J.B. sampled for 421 minutes with the remaining time counted as zero exposure received an 8 hour exposure dose of 98.7 equivalent to an average of 89.8 decibels.  4. T.L. sampled for 431 minutes with the remaining time counted as zero exposure received an 8 hour exposure Dose of 104.0 equivalent to an average of 90.2 decibels.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.
