29 CFR 1910.22(a)(2): The employer did not ensure the floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. When wet processes are used, drainage was not maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats were not provided:   a) Line 1 Trench Conveyor Area in the Front End - On or about January 7, 2020, an employee did not have a dry non slippery floor or surface to walk on inside the trench.

29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazard:   a) Line 1 Trench Conveyor Area in the Front End - On or about January 7, 2020, an employee did not have a dry non slippery floor/surface to walk on inside the trench and the employer did not enforce the use of personal protective equipment for the protection of the body such as but not limited to, slip resistant shoes.
