29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a. Warehouse: On or about February 13, 2020, the employer exposed employees to hazards associated with the operation of forklifts, in that the employer did not provide forklift operators with training to operate forklifts or evaluate prior training of employees who operate Toyota electric forklifts to move pallets of merchandise in the warehouse or to load pallets into trucks for shipping.
