29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) Elliott Building Services LLC on or about February 11, 2020, at the address of 4716 Sunny Creek Ln. Sylvania, OH: The employer failed to ensure employees were protected from a fall while working from a ladder jack scaffold. The employees were exposed to approximately 13 ft. 7 in. fall to the level below during siding installation operations of a new residential structure. No form of fall protection was in place. 29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. a) Elliot Building Services LLC on or about February 11, 2020, at the address of 4716 Sunny Creek Ln., Sylvania, Ohio: The employer did not provide a training program for employees exposed to fall hazards while performing residential construction activities.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system nor are employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) Elliott Building Services LLC on or about February 11, 2020, at the address of 4716 Sunny Creek Ln. Sylvania, OH: The employer did not ensure employees were protected from falls while installing sidings atop a steep sloped residential type new construction structure. Employees were exposed to a potential 13 ft. and 13 ft. 7 in. falls to the level below while performing siding installation activities. No form of fall protection was in place.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder. a) Elliott Building Services LLC on or about February 11, 2020, at the address of 4716 Sunny Creek Ln. Sylvania, OH: The employer did not ensure the side rails of 24ft. aluminum extension ladders used onsite were extended at least three feet above the landing surface. Three employees were exposed to a near 13 ft. fall to the level below while using the ladder to access the north side of the steep slope roof. No form of fall protection was in place.
