29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck: a) Ballyhoo Printing and Design LLC, Print Shop: On or about February 11, 2020 and a times prior, employees were exposed to struck by hazards while operating the Caterpillar Forklift, Model: GP18, Serial: 4AM-00232 to load and unload supply trucks without being trained.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Muller-Martini Baby Pony Perfect Binder, Type 246: On or about November 11, 2020 and at times prior employees were exposed to caught in hazards while conducting tasks such as, but not limited to: removing finished product from the outfeed ramp, and working in and around the cover feed table.
