29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about February 7, 2020, and times prior thereto, employees performing decking work without fall protection on a 7 in 12 pitch roof exposing employees to a fall hazard of approximately 20 feet to the ground below. Jem Michael Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1046195, citation number 1, item number 3 and was affirmed as a final order on May 7, 2015, with a final abatement date of March 11, 2015, with respect to a workplace located at 9100 Old Denton Rd., Fort Worth, Texas 76177.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. On or about February 7, 2020, and times prior thereto, the employer did not provide a certification record to verify training for employees exposed to fall hazards. Jem Michael Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(b)(1), which was contained in OSHA inspection number 1403602, citation number 1, item number 2 and was affirmed as a final order on July 12, 2019, with a final abatement date of June 28, 2019, with respect to a workplace located at 960 Westgate Way, Wylie, Texas 75061.
