29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part. a) The Advanced Construction Group on or about February 11, 2020, at the site location of 5411 Main St. Sylvania, OH: The employer did not have a plan or program to identify job site hazards that assist employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as, but not limited to, falls during job site operations. Two employees were exposed to falls of a roughly 13 feet and 3 inches to the level below while performing roofing activities. No form of fall protection was in place. The employees did not recognize the hazards associated with working from an elevated location without the benefit of conventional fall protection.

During this activity, employees were exposed to falls up to 13 feet and 3 inches to the ground below. No form of fall protection was in place. The employees working from an elevated location without the benefit of conventional fall protection.
