29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition: (a) On or about 02/10/2020 - AC unit on #6 cab, employees were exposed to slip and fall hazards when standing on coal material while performing maintenance on a Lintern condenser.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: (a) On or about 02/10/2020 - AC unit on #6 cab, energy control procedures were not adequate for employees performing maintenance on the Linden HVAC condenser resulting in exposure to caught-in and amputation hazards.
