(a)  On or about February 12, 2020, employees conducting a sewer lateral installation to a residential building in an approximately 12 foot deep trench, were not protected from cave-ins with an adequate protective system.  This exposed employees to struck-by and caught in hazards.    The Bedrock Sewer & water, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1150669, citation number 1, item number 2 and was affirmed as a final order on September 21, 2016, with respect to a workplace located at River Rd. and N. Brookfield Rd., Brookfield, WI 53045.
