29 CFR 1926.416(a)(3): Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit. (a) Randy J. Smith Construction & Roofing, LLC, on or about February 11th, 2020, at the location of 3495 NW Catawba Rd., Port Clinton, OH, 43452: Employees were working in close proximity to live electrical lines which posed an electrocution and shock hazard. The employer did not verify that these lines were inactive before employees began working in this area.

(b) Randy J. Smith Construction & Roofing, LLC, on or about February 11th, 2020, at the location of 3495 NW Catawba Rd., Port Clinton, OH, 43452: The employer did not ensure that employees accessing the upper level of the roof to take measurements using a portable extension ladder were protected from potential fall hazards of approximately 20 feet to the ground below by extending the side rails at least 3 feet above the upper landing.
