29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  Steve Petrick, on or about February 11th, 2020, at the location of Lot 12, McKenna Ln., Port Clinton, OH, 43452:  Employees were performing roofing work on a new residential structure without the benefit of an accident prevention program to identify and mitigate jobsite hazards.  Hazards unmitigated include falls of 10 feet, 4 inches and the lack of a fall prevention training program.
