29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:        On or about February 11, 2020, employees were exposed to falls in excess of 10 feet while accessing and exiting the roof via an extension ladder, which did not extend at least 3 feet above the upper landing surface of the roof, on a commercial construction site in Springfield, Illinois.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:      On or about February 11, 2020, employees were exposed to falls in excess of 10 feet without fall protection, while working outside the warning lines, on a commercial construction site in Springfield, Illinois.       DAN GARREN DBA CYDE ENTERPRISES WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, 1926.501(b)(1O), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1394551, CITATION NUMBER 1, ITEM NUMBER 1a AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 28, 2019, WITH RESPECT TO A WORKPLACE LOCATED AT 512 W SPRESSER STREET, TAYLORVILLE, ILLINOIS 62568.

29 CFR 1926.502(f)(1)(i): When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge:    On or about February 11, 2020, employees were exposed to falls in excess of 10 feet, due to the warning lines not being erected at a minimum 6 feet from the roof edge, on a commercial construction site in Springfield, Illinois.   These instances included but not limited to the following:      a. West side  4 feet, 4 inches and   b. East side - 3.5 feet.    DAN GARREN DBA CYDE ENTERPRISES WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, 1926.502(f)(1)(I), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1242095, CITATION NUMBER 1, ITEM NUMBER 1b AND WAS AFFIRMED AS A FINAL ORDER ON AUGUST 24, 2017, WITH RESPECT TO A WORKPLACE LOCATED AT 420 CROSSING DRIVE, SHERMAN, ILLINOIS 62684.
