29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) Second floor deck, north end, on or about 2-13-20: One employee was installing wood decking/sheathing and was not protected from falls, in that he could fall more than 6 feet due to excess slack in his safety rope, exposing the employee to a fall hazard of 14 feet 8 inches the ground below.

29 CFR 1926.502(d)(16)(iv): Personal fall arrest systems, when stopping a fall, did not bring an employee to a complete stop and/or and did not limit maximum deceleration distance to 3.5 feet (1.07 m): a) Second floor deck, north end, on or about 2-13-20: One employee was installing wood decking/sheathing while connected to a safety rope that was rigged with excess slack, such that the deceleration distance was greater than 3.5 feet, exposing the employee to a fall hazard of 14 feet 8 inches.
