29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about February 4, 2020, and at times prior thereto, in the trim department, employees engaged in bending metal parts with an unguarded Jorns AG, hydraulic trim bending machine, serial number 0178/K15069 were exposed to caught-in and amputation hazards.

29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours. On or about February 4, 2020, at the workplace, the employer did not an employee's work-related amputation to OSHA within twenty- four (24) hours. The employee sustained multiple digit amputation injuries.
