29 CFR 1910.22(a)(2): The floor of each workroom was not maintained in a clean and, to the extent feasible, in a dry condition:    (a) On or about 02/12/2020 - Extruder 1, employees were exposed to slip and fall hazards due to the floor around the extruder machine not being maintained in a dry condition.     (b) On or about 02/12/2020 - Extruder 2, employees were exposed to slip and fall hazards due to the floor around the extruder machine not being maintained in a dry condition and floor mats not being provided.     (c) On or about 02/12/2020 - Extruder 4, employees were exposed to slip and fall hazards due to the floor around the extruder machine not being maintained in a dry condition and floor mats not being provided.     (d) On or about 02/12/2020 - Sorter area, employees were exposed to slip and fall hazards due to the floor being wet from on-going roof leaks.     (e) On or about 02/19/2020 - Grinder area, employees were exposed to slip and fall hazards due to water accumulation in the walkway.

29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average (TWA) of 85 decibels:    (a) On or about 02/19/2020 - Extruder department, the employer did not provide audiograms to employees exposed to an 8-hour TWA of 95.6. decibels. The sampling was performed for 458 minutes during one shift on 02/19/2020.  Zero exposure was assumed for the unsampled period of time of 22 minutes.    (b) On or about 02/19/2020 - Extruder department, the employer did not provide audiograms to employees exposed to an 8-hour TWA of 96.7 decibels. The sampling was performed for 425 minutes during one shift on 02/19/2020.  Zero exposure was assumed for the unsampled period of time of 55 minutes.    (c) On or about 02/19/2020 - Floater department, the employer did not provide audiograms to employees exposed to an 8-hour TWA of 93.5 decibels.  The sampling was performed for 438 minutes during one shift on 02/19/2020.  Zero exposure was assumed for the unsampled period of time. of 42 minutes.    (d) On or about 02/19/2020 - Grinder department, the employer did not provide audiograms to employees exposed to an 8-hour TWA of 96.0 decibels  The sampling was performed for 474 minutes during one shift on 02/19/2020.  Zero exposure was assumed for the unsampled period of time. of 6 minutes.    (e) On or about 02/19/2020 - Sorter department, the employer did not provide audiograms to employees exposed to an 8-hour TWA of 95.3 decibels.  The sampling was performed for 472 minutes during one shift on 02/19/2020.  Zero exposure was assumed for the unsampled period of time of 8 minutes.    (f) On or about 02/19/2020 - Sorter department, the employer did not provide audiograms to employees exposed to an 8-hour TWA of 87.8 decibels.  Sampling was performed for 461 minutes during one shift on 02/19/2020.  Zero exposure was assumed for the unsampled period of time of 19 minutes.

Jet Polymer Recycling, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(c)(2)(i), which was contained in OSHA inspection number 1085048, citation number 2, item number 1 and was affirmed as a final order on 01/21/2016, with respect to a workplace located at 4811 Gault Avenue North, Fort Payne, AL 35967.
