29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: On or about February 13, 2020, at an excavation site located at 5705 15th Street in Midwest City, OK, the employer did not ensure that an employees were in use of protective helmets while working in a trench while storm lines were being lowered into the trench by earthmoving equipment. This exposed the employees to being struck by the hoisted storm lines and falling material.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about February 13, 2020, at an excavation site located at 5705 15th Street in Midwest City, OK, the employer did not ensure that a safe means to access and exit a trench, that was more than 4 feet in depth, was located within 25 feet of the area where employees were working. This exposed the employees working inside of the trench to a fall hazard.
