29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact: A. Jobsite located at 868R Vauxhall Street Extension, Waterford, Connecticut: On, or about, January 28, 2020, a pair of Marigold class 00 voltage rated gloves was used by an employee who performed live electrical diagnostic testing. The last testing date on the gloves was February, 2016. B. Jobsite located in the vicinity of Montville, Connecticut: On, or about, January 30, 2020, personal protective equipment such as, but not limited to, safety glasses had not been provided to an employee who used a Milwaukee drill to drill mounting holes for electrical equipment.
