29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety nets:    At this commericial site: On or a about February 14, 2020, and at times prior thereto, employees were exposed to a fall hazard while installing plywood boards onto the roof at a height over six feet without a fall protection system in place.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  At this commercial building: On or about February 14, 2020, and at times prior thereto, employees without fall protection were exposed to a fall hazard greater than 6 feet to the ground below while roof decking without being trained on fall hazard recognition.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   At this commercial site: On or about February 14, 2020, employees were exposed to a fall hazard, while using a ladder that was less than 3 feet above the upper landing surface for access and egress from the roof.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).  At this commercial site:  On or about February 14, 2020, and at times prior thereto, employees were exposed to a fall hazard while using a Louisville extension ladder for access and egress from the roof which was set six feet away from the building structure.
