29 CFR 1910.101(b): The handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks was not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6. The employer is failing to protect employees from struck-by hazards associated with unsecured storage of a compressed air cylinder in the breakroom/Wildlife Services workstation area. This was most recently documented on February 14, 2020 at 11213 Aurora Ave, Urbandale, Iowa. Instances include, but are not limited to: a. Unsecured cylinder with a regulator attached lying on the floor. Section 3.3.8 of CGA Pamphlet P-1-1965 states "Do not store cylinders near elevators or gangways, or in locations where heavy moving objects may strike or fall on them." b. Employees access the refrigerator in which the door swings towards the unsecured tank. Section 3.3.8 of CGA Pamphlet P-1-1965 states "Do not store cylinders near elevators or gangways, or in locations where heavy moving objects may strike or fall on them."

29 CFR 1910.106(d)(3)(ii): Flammable liquid storage cabinet(s) were not labeled in conspicuous lettering, "Flammable - Keep Fire Away." The employer is failing to protect employees from burn and smoke inhalation hazards associated with storage of flammable liquids in an unlabeled storage cabinet. This was most recently documented on February 14, 2020 at 11213 Aurora Ave, Urbandale, IA. Employees were not provided adequate warning that the metal storage cabinet used to store chemicals may contain flammable liquids. The storage cabinet was not labeled "Flammable - Keep Fire Away."

29 CFR 1910.106(d)(3)(ii)(a): The bottom, top, door, and sides of cabinet were not at least No. 18 gage sheet iron and double walled with 1 1/2 - inch air space. Joints were not riveted, welded or made tight by some equally effective means. The door was not provided with a three-point lock, and the door sill shall be raised at least 2 inches above the bottom of the cabinet. The employer is failing to protect employees from burns and smoke inhalation hazards associated with storage of flammable liquids. This was most recently documented on February 14, 2020 at 11213 Aurora Ave, Urbandale, Iowa. The employer was storing flammable liquids in a single-walled metal cabinet that was not designed to meet minimum fire resistance.
