29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: USS Lake Champlain, Sonar 1: Employees performing hot work operations, such as but not limited to welding during ship repair operations, were required to use half-mask respirators. The employer's exposure determination for respiratory hazards during welding operations did not include conditions similar to those on the ship. Employees were exposed to respiratory irritation hazards associated with welding fumes.

29 CFR 1910.134(h)(1)(i): Respirators issued for the exclusive use of an employee were not cleaned and disinfected as often as necessary to be maintained in a sanitary condition: USS Lake Champlain, Sonar 1: Employees performing hot work and needle gun operations were required to use 3M half-mask respirators. Wipe samples obtained inside employees' respirator masks during the inspection showed the positive presence of the following: a) Iron oxide fume b) Copper fume c) Manganese fume
