29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Fanuc Robot M410IC: On or about 1/24/2020, and at times prior, the employer exposed employees to amputation hazards in that the employer did not develop lockout tagout procedures to control hazardous electrical energy while performing task such as unjamming boxes with a metal rod with a loop at the end.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a) The employer did not notify OSHA of an incident that occurred on or about January 24, 2019, which caused the amputation to the tip of left finger.
