Toledo Refining Company located in Oregon, OH:  On or about February 6, 2020, the employer did not ensure personal air monitoring was conducted for employees working in areas such as but not limited to Plant 2 during a spent catalyst dust release from the FCC catalyst regenerator upstream snort outlets.  The spent catalyst dust contained a number of hazardous substances and heavy metals and was released over 60 to 90 minutes on two separate days.
