Sylvania, OH: The employer did not have a plan or program to identify job site hazards that assist employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as, but not limited to, falls during job site operations.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. a) R L R Graber Construction, LLC, on or about February 11, 2020, at the site location of 9449 Sweetwater Ln. Sylvania, OH: Two employees were observed lifting and stacking materials that are piled, grouped, or stacked near a roof edge atop the second level of a new residential construction structure. The employer did not ensure that a fall protection training program was in place for one employee that was exposed to fall hazards over six feet in residential construction.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels is not protected by guardrail systems, safety net system, or personal fall arrest system nor are employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) R L R Graber Construction, LLC, on or about February 11, 2020, at the site location of 9449 Sweetwater Ln. Sylvania, OH: The employer did not ensure employees were protected from falls while installing framing atop a second level residential type new construction structure. Seven employees were exposed to a potential 12 ft, fall to the level below while performing framing activities. No form of fall protection was in place.
