The employers program did not identify the machines hydraulic energy sources.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed. On or about February 12, 2020, the employer failed to conduct an audit of their control of hazardous energy program to ensure that the program procedures are up to date and effective. The last review of this program was conducted in 2018 and since that date, there have been new employees and systems put into service, to include adding locking out the hydraulic energy source on the presses. Presses to include but not limited to the Trumpf Press Brake (serial#50504A0465).

Press operators were not trained as authorized to perform tasks such as die change outs and set up processes.
