29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  On or about February 18, 2020, and times prior thereto, employees operating an unguarded Leeson belt sander to sand metal material were exposed to being caught-in the rollers and crushing, amputation hazards.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s).  a) On or about February 18, 2020, and times prior thereto, employees operating the horizontal band saw (Model: Antares, Series: D02107) without point of operation guarding exposing employees to caught-in and amputation hazards.  b) On or about February 18, 2020, and times prior thereto, employees operating two (2) Roll-In vertical band saws without point of operation guarding exposing employees to caught-in and amputation hazards.
