29 CFR 1910.28(b)(1)(i): Each employee on a walking/working surface having an unprotected side or edge which was four feet (1.2m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about 2/18/2020, in the Chateau Villa located at 2031 Morganton GA: Employees performing servicing and maintenance activities on top of the cages were exposed to a fall hazard in excess of 15-feet, where the employer failed to install a guardrail system and or ensure the employees used a fall arrest system.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about 2/18/2020, in the Chateau Villa and other Villas located at 2031 Lowery Road, Morganton, GA: Employees performing servicing and maintenance activities on the overhead fans, lights, and ventilation system were exposed to amputation and struck-by hazards, in that; no procedures were developed or utilized to control the electrical and rotating energy sources such as but not limited to the following equipment. a. Employer did not developed procedure for overhead fans - Electrical 120 volts energy source. b. Employer did not developed procedure for overhead lights - Electrical 120 volts energy source. c. Employer did not developed procedure for ventilation systems - Electrical 120 volts energy source. d. Employer did not developed procedure for pump house systems - Electrical 220 volts energy source.
