29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about February 13, 2020, an employee was framing a house while standing on a board approximately twenty-five (25) feet off the concrete floor below and was not protected from a fall hazard. (b) On or about February 13, 2020, an employee was framing a house while standing on top of a second story wall approximately twenty (20) feet off the concrete floor below and was not protected from a fall hazard. (c) On or about February 13, 2020, employees were framing a house while accessing/working from the first level decked floor approximately ten (10) feet off the concrete floor below and were not protected from a fall hazard.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed; the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: (a) On or about February 13, 2020, employees were using a Werner multipurpose ladder placed against the header of the first level floor and its side rails did not extend passed the landing at all exposing them to a fall hazard.
