29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.   a. On or about February 7, 2020, the employer did not develop or document machine specific energy control procedures for employees that were engaged in service and maintenance activities on equipment, such as, but not limited to, DES and SES line auxiliary equipment, motors, and pumps, where the unexpected energizing, start up or release of stored energy could occur and cause injury.

29 CFR 1910.212(a)(5): Exposure of blades. When the periphery of the blades of a fan is less than seven (7) feet above the floor or working level, the blades shall be guarded. The guard shall have openings no larger than one-half (1/2) inch.  a.  On or about February 7, 2020, located at the backside of the SES line, two motor cooling fans did not have a guard installed, exposing employees to an amputation injury.  The fans were approximately 12 inches from the floor level.    b. On or about February 7, 2020, located at the backside of the DES line, a motor cooling fan did not have a guard installed, exposing employees to an amputation injury.  The fans were approximately 32 inches from the floor level.

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.  a. On or about February 7, 2020, located near the end of DES line, a relocatable power tap was used as a substitute for fixed wiring for supplying power to hazardous chemical transfer pumps. The relocatable power tap was not used in accordance with the UL listing and exposing employees to a potential fire hazard.

29 CFR 1910.303(b)(7)(iv): There shall be no damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.  a. On or about February 7, 2020, in the HAL room, there was a 480 volt disconnect that supplied electrical power, in part to the HAL pre-clean machine. The electrical disconnect switch located on the exterior of the electrical panel was broken off, which did not allow safe operation if an emergency occurred with the equipment and electrical power had to be immediately removed.

29 CFR 1910.303(g)(1): Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.   a. On or about February 7, 2020, in the HAL room behind accumulator #8, there was a 480 volt disconnect that supplied electrical power, in part to the hot air solder leveling equipment. The electrical disconnect was blocked by accumulator #8, stored chemical containers, and supply boxes. These blockages did not allow access to the electrical disconnect if an emergency occurred with the equipment and electrical power had to be immediately removed.
