29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 2/12/20, at a 2-story building at 113 Lake Ave, Blasdell, New York: Two employees were installing a metal roofing system on the steep sloped roof on the west side of the building. The employees were exposed to a fall of approx. 20 ft. because no fall protection system was utilized. The pitch of the roof was approx. 14 in 12. NO ABATEMENT CERTIFICATION REQUIRED b) On or about 2/12/20, on the garage roof at 113 Lake Ave, Blasdell, New York: Two employees were installing a metal roofing system on the steep sloped roof on the north side of the garage. The employees were exposed to a fall of approx. 8 to 16 ft. because no fall protection system was utilized. The pitch of the garage roof was approx. 10 in 12. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 2/12/20, at a 2-story building at 113 Lake Ave, Blasdell, New York: An extension ladder was set up on the ground on the west side of the building, and used by employees to access the roof. The siderails of that extension ladder were approx. 8 inches above the roof. NO ABATEMENT CERTIFICATION REQUIRED
