An employee hired in August, 2019 and others hired since training last occurred in 2016, have been operating forklifts without training.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: In the facility at 35 Turnpike Industrial Road, Westfield, MA; On and before February 19, 2020, the employer failed to provide the basic advisory information on respirators to employees allowed to voluntarily wear face masks. An employee working near deburring areas was permitted to wear a face mask to protect from nuisance metal dust without the information presented in Appendix D of 29 CFR 1910.134.
