29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck:  In Buildings 1 and 3, Headquartered at 35 Turnpike Industrial Road, Westfield, MA; On and before February 19, 2020, the employer did not ensure that each employee had been trained prior to operating a powered industrial truck. An employee hired in August, 2019 and others hired since training last occurred in 2016, have been operating forklifts without training.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  In Buildings 1 and 3, Headquartered at 35 Turnpike Industrial Road, Westfield, MA; On and before February 19, 2020, triennial evaluations were overdue for all powered industrial truck (PIT) operators.  At least one  PIT operator had not been evaluated since 2014 and at least seven PIT operators had not been evaluated since 2016.

At the establishment headquartered at 35 Turnpike Industrial Road, Westfield, MA; On and before March 2, 2020, the employer was required to electronically submit information from OSHA Form 300A but failed to do so.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  In the facility at 35 Turnpike Industrial Road, Westfield, MA; On and before February 19, 2020, the employer failed to provide the basic advisory information on respirators to employees allowed to voluntarily wear face masks.  An employee working near deburring areas was permitted to wear a face mask to protect from nuisance metal dust without the information presented in Appendix D of 29 CFR 1910.134.
