29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. a) Ruby Haulage, Inc. located at 5013 Dierker Road, Columbus, OH 43220: On and preceding February 19, 2020, the employer did not ensure that excavated or other materials were kept at least 2 feet from the edge of an excavation that employees were working in. Two employees were installing water lines in an excavation with the spoils pile and other materials sitting at the edge of the excavation. The following condition exposed the employees to struck-by and caught-in hazards.
