29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   Worksite located at 210 Ramblewood Drive, Raymore, Missouri- Employees were exposed to potential eye injuries while using hand staplers as well as pneumatic nail guns and were not wearing any means of eye protection.  The Angel Roofing, LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1) which was contained in OSHA inspection number 1331705, citation number 1, item number 4 and was affirmed as a final order on 12/21/2018, with respect to a workplace located at 21517 W. 98th St., Lenexa, KS 66220

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a)  NOTE TO CSHO: USE THIS OPTION ONLY IN CASES WHERE THE INFEASIBILITY/GREATER HAZARD EXCEPTION IS NOT IN ISSUE.  Worksite located at 210 Ranglewood Drive, Raymore, Missouri- Employees were exposed to fall hazards while working on a steep roof and were not protected from falling by the use of any means of fall protection.  The Angel Roofing, LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1331705, citation number 1, item number 4 and was affirmed as a final order on 12/21/2018, with respect to a workplace located at 21517 W. 98th St., Lenexa, KS 66220.  The Angel Roofing, LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1350707, citation number 2, item number 1,  and was affirmed as a final order on02/15/2019, with respect to a workplace located at 17714 Penrose Lane, Lenexa, KS 66219.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Worksite located at 210 Ramblewood Drive, Raymore, Missouri- Employees were exposed to fall hazards in that the employees were using a ladder that was not extended at least 3 feet above the upper landing surface to access the roof.  The Angel Roofing, LLC.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1350707, citation number1, item number 1and was affirmed as a final order on 2/12/2019, with respect to a workplace located at 17714 Penrose Lane, Lenexa, KS 66219.
