29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers. (a) Integrated Water Services, Inc. at 1600 12th St. Denver, CO 80204: On or before 2/12/2020, the correcting and exposing employer did not protect employees and subcontractor employees from injury in that subcontractor employees were setting rebar and formwork around nine unguarded manholes. The rigid manhole covers were removed prior to commencing with setting rebar, formwork, and concrete. No other methods of affixing a rigid cover were performed. This condition exposed employees and other subcontractors to fall and drowning hazards.

29 CFR 1926.502(i)(2): Covers were not capable of supporting, without failure, at least twice the weight of employees, equipment, and materials that may be imposed on the cover at any one time: (a) Integrated Water Services, Inc. at 1600 12th St. Denver, CO 80204: On or before 2/12/2020, the correcting and exposing employer did not protect employees and subcontractor employees from injury in that subcontractor employees were setting rebar and formwork around nine unguarded manholes. The rigid manhole covers were removed prior to commencing with setting rebar, formwork, and concrete. Plastic sheeting was placed over the manholes and a cover capable of supporting at least twice the weight of employees was not installed. This condition exposed employees and subcontractor employees to fall and drowning hazards.

The rigid manhole covers were removed prior to commencing with setting rebar, formwork, and concrete. Plastic sheeting was placed over the manholes and were not marked in accordance with this standard. This condition exposed employees and subcontractor employees to fall and drowning hazards.
