29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. The employer is failing to protect employees from the electrical hazards associated with improper wiring techniques and routing of wires where potential for abrasion could occur. This was most recently documented on February 20, 2020 Avamere at Lexington 1811 Ridgeway Drive Lexington, NE 68850. The cord itself was subject to abrasion as it was routed through an opening without grommet or clamping device to secure.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. The employer is failing to ensure that electrical outlets are covered with faceplates. This was most recently documented on February 20, 2020 at the facility of Avamere at Lexington, Lexington, NE 68850. A faceplate was not installed at an outlet in the maintenance office that was live, and supplying power to overhead light fixture.

29 CFR 1910.334(a)(3)(ii): Attachment plugs and receptacles may not be connected or altered in a manner which would prevent proper continuity of the equipment grounding conductor at the point where plugs are attached to receptacles. Additionally, these devices may not be altered to allow the grounding pole of a plug to be inserted into slots intended for connection to the current-carrying conductors. The employer is failing to protect employees from the shock hazard associated with altered electrical cords. This was most recently observed on February 20, 2020 at the facility located Avamere of Lexington. Lexington, NE 68850. An overhead light assembly power cord was determined to be a modified HIGH VIS Orange extension cord, direct wired to light.
