Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. a) On or about 02/19/2020 there was no frequent and regular inspection program being maintained in use by a competent person to ensure the job site, material and equipment; such as fall protection system was being utilized.

29 CFR 1926.451(g)(1): Fall protection. Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. a) On or about 02/19/2020 An employee was sitting on top of a scaffold system installing siding; with no fall protection system in place. The height of the fall hazard was measured at 13.5 ft. from the ground.

29 CFR 1926.454(a)(1): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: The nature of any electrical hazards, fall hazards and falling object hazards in the work area; a) On or about 02/19/2020, an employee was not trained to recognize fall hazards such as, but not limited to, a 13.5 ft. fall hazard while working on top of a scaffold system, nor was the employee trained on when to utilize a fall protection system.
