29 CFR 1926.651(b)(1): The estimated location of underground utility installations, such as sewer, telephone, fuel, electric, water lines, or any other underground installations that reasonably may be expected to be encountered during excavation work, were not discovered prior to opening an excavation: (a) Levi Shelby Plumbing, LLC at 980 68th Pl. Denver, CO 80221: On or before 2/20/2020 the employer did not ensure that underground utilities were located and marked prior to opening the excavation. An excavation 17 feet long by 11 feet wide and 12 feet deep was opened without first locating utilities.
